time. Our team of professionals, caregivers and trained
volunteers are dedicated to answering your questions
and providing medical and emotional support to you
and your loved one. We encourage the patient and family
members to express their needs and actively participate
in the decision-making process. In addition to the
services listed below, a registered nurse is available
on-call 24 hours a day, 7 days a week for phone consultation
or visit if needed."

The attending
physician participates in and authorizes the ongoing
development and revisions of the plan of care.
The Hospice Medical Director approves Hospice admissions,
provides overall medical supervision to Hospice patients
and may provide medical care to individual patients
when appropriate. Good Shepherd Hospice’s
physicians are board certified in Hospice and Palliative
Care
1. Certifies
patient’s medical eligibility for Hospice care.
2. Provides medical supervision by participation in
the interdisciplinary team care plan development,
monitoring and revision.
3. Provides for pain and symptom management of Hospice
patients when appropriate. (See/Print our PAIN
MANAGEMENT SCALE for reference)
4. Provides education and consultation to other physicians
and the Hospice team on pain, symptom management and
hospice care.
5. Makes home visits to patients as appropriate.
Nursing
services are provided by a registered nurse trained
and educated to meet the needs of the terminally ill.
A primary nurse is assigned to each patient/family
unit and is responsible for coordinating the patient’s
care in all settings. The professional nurse participates
in the formulation, evaluation and modification of
the interdisciplinary plan of care.
1. Nursing assessment
of patient/family needs and ongoing evaluation.
2. Participation in the interdisciplinary care plan
development, monitoring and revision.
3. Intermittent visits on a scheduled and as needed
basis.
4. May provide extended hours of nursing care when
continuous nursing care is evaluated as necessary
by the interdisciplinary team.
5. Provide medical treatments, pain management
and symptom control in collaboration with a physician.

6. Teaching regarding Hospice care and services, care
techniques, disease progression and other health measures
to patient, family and care giver.
7. Providing supportive care and counseling.
The licensed practical nurse may provide nursing care
in accordance with the interdisciplinary plan of care
with supervision by the patient’s primary professional
nurse.
The Social Worker
provides for the psychosocial needs of the patient/family.
Good Shepherd Hospice’s Social Workers are all
licensed and have earned their masters degrees.
1. Assessment and ongoing
evaluation of the social, psychological and emotional
factors related to the patient’s terminal illness
and the family’s response.
2. Participation in the interdisciplinary team care
plan development, monitoring and revision.
3. Bereavement assessments.
4. Individual, family and group counseling and support.
5. Assistance with securing insurance coverage for
services, financial and community resources as needed.
It is within
the tradition of Good Shepherd Hospice to provide
spiritual, emotional and physical support to those
who are experiencing a life-limiting illness and to
their loved ones.
The journey need not be a lonely one. Our Chaplains
are available to WALK WITH YOU.
Good Shepherd Hospice Chaplains are available to visit
you and your family to offer spiritual and emotional
support. Our Chaplains have been trained to visit
persons of all faith traditions, to listen and to
reflect with you. They are available to help you in
contacting your own Pastor, Rabbi or Faith Community.
If you want someone to talk with, pray with or just
be with our Chaplains are available. They may be reached
by calling the Hospice office or by asking a Good
Shepherd staff member to forward your request if you
are already on our program. Good Shepherd Hospice’s
Spiritual Care staff are Certified as Chaplains.
The Home
Health Aide provides personal care to hospice patients
under the supervision of the registered nurse.
1. Assisting
the patient or performing activities of daily living,
light housekeeping, shopping and the completion of
errands at home or in a facility as needed.
2. Reporting to the registered nurse any changes in
the patient’s condition as directed by the nurse.
3. Providing compassionate comfort care to the patient
which relieves the care giving family.
- A registered dietician to provide nutritional services for hospice patients as needed.
- Physical, Occupational, Speech and Respiratory Therapy
Medical Equipment is provided as needed for patient safety and comfort. Lab services are provided as ordered by the physician.
Good Shepherd’s goal is the comfort of the patient and convenience for the family. Our staff members will help to coordinate the delivery of prescription drugs for pain management and symptom control as well as durable medical equipment and most routine laboratory services can be done at home. All Medications related to the terminal disease are provided by Hospice 24 hours per day at no cost to the patient or family
The Hospice social worker and nurse will be available to meet with you and discuss possible care options for the patient. Each situation is unique and it is our goal that each discharge plan be individualized.

1. Your doctor has been involved and agrees that inpatient care is appropriate.
2. Would the patient’s needs be best met at home or in a skilled care hospital environment?

Hospice Bereavement Program
Experience has taught us that people coming together
to share their thoughts and emotions in a safe environment
are crucial to the mourning process.
All bereavement services are provided by a Social Worker who is licensed and has earned a masters degree in the profession.
Our professionally trained staff provides bereavement services to hospice families for a period of 13 months after a loss. Additionally, the Good Shepherd Hospice Bereavement Program offers the following non-sectarian services without fees to all residents of Nassau and Suffolk Counties:

- Office Visits for Individual Counseling
- Bereavement Home Visits
- Spouse Bereavement Support Group
- Adult Bereavement Support Group (For Losses Other Than Spouses)
- Children’s Bereavement Support Group (Age Appropriate Support Groups)
