TITLE: Home Health Aide Inpatient Center
REPORTS TO: RN Manager
LOCATION: Port Jefferson
HOURS: F/T – Days, Evenings & Night positions available. Night differential offered.

POSITION SUMMARY:

Provides personal care, under the supervision of the Registered Nurse and assists the patient in the achievement of optimal physical and emotional comfort at the Good Shepherd Hospice Inpatient Center.

PRINCIPLE DUTIES AND RESPONSIBILITIES:

  1. Provides personal care services to an assigned patient caseload in accordance with the plan of care established by hospice nurse and interdisciplinary care team.
  2. Performs simple physical care procedures taught and assigned by a hospice nurse, i.e., this shall include but not be limited to: bed baths, changing bed linens, showers, tub baths, and shampoos, oral care, routine skin care, hair and nail care (non-diabetics), perineal care, and shaving, as indicated by the plan of care. Assists patients with elimination needs including bed pans, urinals, incontinence care and/or toileting.
  3. Observes, reports and documents changes in the patient’s status promptly to the RN or Charge Nurse.
  4. Answers ANY/ALL call lights and attends to/reports patient’s needs.
  5. Applies safety principles and proper body mechanics to the performance of specific techniques of personal and supportive care. Assists patients in and out of bed, chairs, and stretchers; lift/turn and position patients frequently according to care plans, applying the principles of good body alignment.
  6. Assists patients with feeding/meals including transporting from patient room to other hospice areas, distribution and collection of trays. Distribute drinking water and nourishments.
  7. Assists with admissions, transfers and discharges as requested by the RN.
  8. Assists with maintaining a safe, clean, and orderly environment.
  9. Maintains patient’s living area consisting of bed, bedside table, overhead table, wheelchairs, geriatric chair, clothing drawers, closets, and bathroom storage area in a clean, orderly manner.
  10. Maintains non-patients areas (tub/shower rooms, utility rooms, etc) in a neat and orderly condition as assigned.
  11. Operates all equipment and performs all procedures/care in a safe manner. Demonstrates proper body mechanics in all functions. Cleans and stocks supplies and equipment as directed.
  12. Performs errand services for nursing staff as needed. Performs related and miscellaneous duties as directed. In an emergency situation, may be required to perform unrelated duties.

KNOWLEDGE, SKILLS AND ABILITIES:

  1. Satisfactory completion of a Home Health Aide Training Program approved by the New York State Health Department with minimum 1 year recent experience.
  2. Basic understanding and commitment to the hospice concept of care for the terminally ill.
  3. Presents neat appearance in proper attire and identification as required by Good Shepherd Hospice policy.
  4. Ability to deal with own and patient/family stress, no significant family losses in past year.
  5. Verbal and written skills in easily understood English.
  6. Valid NYS Drivers license.
