JOB POSTING

TITLE: F/T Hospice Admission Nurse
REPORTS TO: Director of Access

SUMMARY OF DUTIES:

  1. Exhibits good understanding of Hospice philosophy and expertise in the care of the terminally ill.
  2. Knowledgeable regarding the Hospice Medicare benefit, as well as understanding of other insurances.
  3. Ability to communicate in sensitive, professional and empathetic manner to family and individual facing end of life decisions.
  4. Communicates with physician and obtains appropriate medical orders to enhance patient comfort and symptom control.
  5. Knowledgeable on pain/symptom management and end of life social/spiritual concerns.
  6. Communicates patient/family needs to appropriate members of IDG within 24 hours.
  7. Coordinates equipment, medications, Home Health Aide and other services needed by patient/family for optimum care and safety.
  8. Completes documentation within 24 hours of admissions in a professional legible manner.
  9. Makes routine nursing visits as requested.
  10. Participates in on-call rotation as requested.
  11. Takes telephone referral and follow through to patient admission if requested.
  12. Strong organizational skills and flexibility to work the hours necessary to enable Hospice top meet its commitments to the patients / residents and families. 
  13. Willingness to travel to patients’ homes and provide own transportation in all types of weather. Possesses a current, valid New York State driver’s license.
