The Hospice On-Call Nurse will be available to provide
nursing care and support to hospice patients and/or
families on an on-call basis (evenings, nights, weekends
and holidays) within their home setting. The care provided
is consistent with the hospice interdisciplinary plan
of care and focuses on symptom management and pain control.
POSITION SUMMARY:
Exhibits good understanding of Hospice philosophy and
expertise in the care of the terminally ill patient
/ resident.
Assess and responds to the needs of terminally ill
patient / resident, insuring appropriate clinical intervention,
communicating pertinent information to the physician,
and offering support to the patient and family.
Gives instructions and reassurance as needed. Makes
home visits if a problem is unable to be resolved by
phone and cannot wait for a home visit during working
hours.
Coordinates services during on-call hours such as emergency
medications, equipment, supplies, aide service, inpatient
care, and other services available from hospice.
Supervises personnel (direct and contract) during on-call
hours, i.e., home health aides and continuous nursing
staff.
Interacts with Hospice team members and communicates
appropriate information regarding status of the patient
/ resident.
For patients and families not active with hospice,
ensures that caller receives correct information as
to routine business hours. Directs callers to other
resources if an emergency exists.
Completes progress notes and log sheets to document
assessment, intervention and follow up for patients
and families. Documentation is submitted in a timely
manner
Willingness to travel to patients’ homes and
provide own transportation in all types of weather.
Possesses a current, valid New York State driver’s
license
Current registration in New York State as a Licensed
Nurse and one to two years recent general bedside nursing
experience with good clinical skills. Experience in
home care, hospice and/or oncology preferred.
