SUMMARY OF DUTIES:
Evaluates the resident’s / patient’s /
family’s individual needs for physical, emotional,
social and spiritual; care. With the other Hospice
team members, develops an individualized, comprehensive
plan of care for each patient / resident /family.
Provides skilled care to patients / residents requiring
intermittent professional nursing service. Evaluates
the need and arranges for more intensive nursing or
home health aide care as appropriate. Documents all
care in an appropriate and timely manner.
Coordinates Hospice services for individuals and families
under his/her care. Maintains necessary records for
analysis and planning of service and for the establishment
of priorities of care. Initiates referrals to appropriate
disciplines and outside resources and works with them
to provide continuing and coordinated care.
Teaches, supervises and interprets care plans to family
members, facility staff, auxiliary nursing personnel
(Home Health Aides) or others giving nursing care (family
employed substitutes). Assists family in planning for
the provision of care between Hospice Primary Care
Nurse visits.
Attends weekly team meeting to share, review and revise
individual patient / resident / family care plans.
Responds to calls received by answering service from
active patient/families and bereaved families during
assigned hours. By phone, assess patient/family
needs.
Gives instruction and reassurance as needed. Makes
home visits if a problem is unable to be resolved by
phone and cannot wait for a home visit during working
hours
Coordinates services during on-call hours, such as,
emergency medications, equipment, supplies, aide services,
inpatient care, and other services available
Supervises hospice personnel (direct and contract)
during on-call hours, i.e. home health aides and continuous
nursing staff
Communicates with physicians and other team members,
as needed, to obtain orders and advise of changes of
status and interventions taken
Completes progress notes and log sheets to document
assessment, intervention and follow up for patients
and families. Paperwork is submitted in a timely manner
Willingness to travel to patients’ homes and
provide own transportation in all types of weather.
Possesses a current, valid New York State driver’s
license
RN Graduate from an NLN-Accredited nursing program.
Baccalaureate preferred.
Minimum of 1-2 years of RN experience
