HOSPICE ADMISSIONS

One call puts you in touch with the Good Shepherd Hospice Referral & Admissions Department where a registered nurse coordinates your hospice needs. Patients, families, friends or health care professionals may initiate referrals.

Criteria for Admission
  • Patient and family desire hospice care
  • Patient has a life expectancy of six months or less if the disease follows its normal course
  • Patient is not receiving curative therapies
  • Patient has a primary physician who will continue to provide care, and will certify the patient’s prognosis
  • Patient has a primary caregiver
  • The home environment is suitable for meeting the patient’s needs.

Why is Hospice Different from Homecare?
  • The focus is on palliation rather than cure
  • The patient need not be homebound
  • The patient and family are the focus of care
  • Medications related to the terminal illness for pain and symptom control, provided at no cost and often delivered to the home
  • The plan of care is developed and implemented by an Interdisciplinary Team
  • In home continuous care nursing is provided in time of crisis
  • Bereavement support for 13 months
  • 24 hr on-call RNs for telephone support or emergency visits

In Assessing A Patient for Hospice, Ask?
  • Would you be surprised if the patient was not here in 3-6 months?
  • What is the prognosis and co-morbidities?
  • What is the understanding of the patient and family regarding the diagnosis and prognosis?
  • Has curative treatment been completed and goals are now centered on comfort and symptom control?
